How do I know if I'm eligible?

Eligibility is specific to your insurance plan. To verify that your individual coverage qualifies, please call the phone number on the back of your health insurance card to receive your Fitness ID or Confirmation Code for the program. Once the eligibility has been verified, obtain your Fitness ID or Confirmation Code as this will be required to enroll in membership.

How do I enroll and what's next?

You must visit your Life Time location to enroll in this program. Once you have received your Fitness ID or Confirmation Code and verified your Life Time location accepts the program, you are welcome to stop in to the club during guest hours or call your club directly at 855-430-5433. During normal business hours, a Life Time representative can also assist you via Live Chat. Simply click on the chat icon if it appears at the bottom corner of this page.
